30. Tophaceous gout

The nodules on this patient’s finger, ear, toe, and external maleolus are tophi. Diagnostic proof emerged when the tophus on her finger discharged chalky-white material containing needle-shaped sodium urate crystals that were negatively birefringent under polarized light microscopy.
