adding an exercise inside an example inside an example for testing of ticket:6275

Adding a bunch of empty cnxns (and some IDs to other elements to which to connex them) for testing, as well as a few other minor changes, probably leftover from testing accessibility things.

Added an example of how media that appear to be the same size on screen can be a different size in print due to resolution.

Added text to the note with a type of FOOTNOTE to be more clear about the difference in styling between it and other notes with type of footnote (all lower-case).

oops, i checked out the wrong module, so i didnt really make any changes to this one.  just deleted a space.

changed the target of an empty cnxn tag to see if it would display the target correctly after the XSL transformation

Added an equation as a descendant of definition as a child of item.

updated this module to CNXML 0.4 by adding the md namespace and removing now invalid arrangements of tags (primarily list within objectives)

Made a note without an attribute, and added Brent and Yasushi as maintainers.
